Write a detailed damage report about the course of events, including the following information:

• Who was acting as the boat's captain?
• Who was steering the vessel?
• What were the weather conditions like?
• Where was the vessel coming from and where was it going?
• Exact location of the damage site on a nautical chart or equivalent
• Course and speed in the damage situation
• How was navigation performed and who was responsible for it?
• What was the water depth?
• Is the shallow area marked on the chart?
• Was the damage noticed during the journey? If yes, how was it reacted to?

• Photos of the damage
• Names and addresses of parties involved
• Names, addresses and statements of witnesses
